Some services provided by the practice are not covered by the NHS and you will be asked to pay a fee. Chargeable services include:

• Medical examinations (pre-employment, HGV, elderly driver etc.)
• Medical sickness and fitness to travel certificates
• Forms (e.g. driving licence, fitness to participate in certain sports)
• Some travel vaccinations and certificates
• Insurance claim forms 
• Some private medication including malaria prophylactics
• Access to medical records
• Other letters or forms as requested
